Make "house" calls to a private residence, a residential care facility, or where you are living with family. When an in-person visit is inconvenient, we offer telehealth phone appointments or appointments on a secure video platform as well.
Participate in transitional care as the medical office responsible for patient care coordination. This keeps you making progress toward coming home instead of digging through hospital bureaucracy.
Make referrals to medical specialists for chronic conditions as needed to ensure you receive the quality care you deserve, and we continue with follow-up care.
Provide coordinated care with your assisted living facility. Each facility tends to have a different suite of services, so we will coordinate our services with theirs.
