Not all residential care homes have consistent medical staff to provide a full suite of skilled medical services. In many cases, attention from a primary care doctor or a nurse practitioner (NP) may be lacking. This leaves a vital gap to be filled by primary care providers who come to you. We fill that gap, including less common services such as wound care.
COVID-19 has changed a lot about the world. One positive change, however, has been a burst of innovation in medical care, especially primary care, by using telehealth visits, often called remote patient monitoring services. This is when telephone calls or video calls through smart phones, tablets, or computers connect you with your healthcare provider between visits so we can check on your progress, and determine if a physical visit is necessary. Medicare covers telemedicine visits.
The most difficult part of a chronic medical condition is that it is there all the time. Ongoing medical conditions are best treated by a primary care physician and team who are also consistently available. While we do not provide 24/7 in-home care, we have built our in-home assistance model to provide you with frequent care by your primary care physician. You do not have to be alone in your chronic condition struggle. Medicare covers chronic care services.
Senior Transitional Care
If you or a loved one have recently been hospitalized, there may be a gray area in which you do not need inpatient hospital care, but you are also not quite ready to be on your own. This is when a senior transitional care center comes in. These centers are designed to help you become strong enough to go home with the confidence that you will be safe in doing so. Elevate Healthcare works with these centers to coordinate the transition and, if desired, to continue as your healthcare provider once you are back home.
